Flecarite tablets 100mg contain flecainide an antiarrhythmic that is used for the treatment of arrhythmias of the upper (atria or supraventricular) chambers of the heart and lower (ventricular) chambers of the heart.  

Flecarite general information

What is Flecarite used for?

Flecarite tablets 100mg are used to treat and prevent severe heart rhythm disorders or arrhythmias, in patients without structural heart disease and without myocardial infarction (heart attack).  These conditions are due to irregular electrical activity of the heart and include arrhythmias of the upper (atria or supraventricular) chambers of the heart and lower (ventricular) chambers of the heart.  Flecarite Tablets 100mg are used to treat severe paroxysmal supraventricular tachycardias (PSVT), where other treatments have not worked or been tolerated.  Paroxysmal indicates that the episode of arrhythmia begins and ends abruptly.  Other forms of supraventricular tachycardia (SVT) include atrioventricular nodal or re-entrant tachycardia, and atrioventricular re-entrant tachycardia and other supraventricular tachycardia of unspecified mechanism associated with disabling symptoms.  The term re-entrant refers to the fact that there is an extra electrical pathway, known as an accessory pathway, that can form a short circuit within the heart’s conduction system, causing the electrical impulse to continue instead of fade as in a normal heartbeat.  The two most common types of SVT are atrioventricular (AV)-nodal re-entry tachycardia and AV re-entry tachycardia (AVRT), which is associated with Wolff-Parkinson-White syndrome (WPW).  Flecarite Tablets 100mg are used also used to treat ventricular arrhythmias such as sustained ventricular tachycardia (VT) but only if they are considered treat life-threatening.   If the ventricles contract too fast or irregularly, the heart cannot pump efficiently and this can become life-threatening, because the supply of blood and therefore, oxygen to the heart itself and the rest of the body is compromised.  This increases the workload on the heart, which also increases the demand for oxygen and can result in ischemia (restriction of oxygen to the tissues). Flecarite Tablets 100mg are used also used to treat paroxysmal atrial arrhythmias, include atrial fibrillation, (which is the most common form of arrhythmia), atrial flutter, (characterised by fast and/or irregular heartbeat due to abnormal electrical discharges in the atria) and atrial tachycardia (rapid heartbeat) in patients with disabling symptoms.  If the atria contract too fast or irregularly, they cannot completely empty of blood and this increases risk of blood clots, which can increase risk of heart failure and stroke.  Flecarite Tablets 100mg are not intended for use in chronic atrial fibrillation. 

How does Flecarite work?

Flecarite Tablets 100mg contains flecainide an antiarrhythmic that belongs to the membrane stabilising (Class I) group of antiarrhythmic drugs and is used for the treatment of severe symptomatic, life-threatening ventricular arrhythmias and supraventricular arrhythmias.  Flecainide in Flecarite Tablets 100mg also has local anaesthetic activity and electrophysiological effects characteristic of the IC (fast inward sodium channel blockers) class of antiarrhythmics.  The main mechanism of action by which flecainide in Flecarite Tablets 100mg exerts its antiarrhythmic effect is by blocking the fast sodium channels that allow sodium ions into the cardiac cells, slowing down electrical conduction in the ventricles, with the greatest effect on bundles of nerve fibres in the ventricles known as His-Purkinje system, which determines the intraventricular (H-V) conduction time.  The His-Purkinje system is composed of specialized cells and nerve fibres that transmit impulses from the atrioventricular node of the heart and are responsible for the synchronous activation of the ventricles so that they contract evenly.  This action of flecainide in Flecarite Tablets 100mg decreases intracardiac conduction in all parts of the heart and prolongs the time to the first recurrence as well as the interval between recurrences of supraventricular tachyarrhythmias; as well as can suppressing recurrence of ventricular arrhythmias. 

What does Flecarite contain?

What are the side effects of Flecarite?

The most commonly reported side effects when taking Flecarite Tablets 100mg include, nausea, vomiting, diarrhoea, constipation, abdominal pain, asthenia (weakness), fatigue, pyrexia (fever), oedema, dyspnoea (shortness of breath), headache depression, anxiety, insomnia, dizziness, light headedness, giddiness, paraesthesia (tingling sensation), ataxia (loss of coordination), dyskinesia (involuntary movements), hypaesthesia (reduced sensation), hyperhidrosis (increased sweating), syncope (fainting), tremor, vertigo, flushing, somnolence (sleepiness), insomnia, tinnitus, visual disturbances (double vision and blurring).  Pro-arrhythmic effects are common but most likely in patients with structural heart disease and/or significant left ventricular impairment, and include new or exacerbated ventricular arrhythmias and new or worsened congestive heart failure.

When should Flecarite not be used?

You should not use Flecarite Tablets 100mg if you:

  • are allergic to Flecainide or any ingredients in Flecarite
  • are pregnant or are breastfeeding
  • have kidney or liver disease
  • have structural heart disease (ventricular dysfunction or hypertrophy, history of myocardial infarction, symptomatic IHD, valvular heart disease)
  • have other heart conditions including second or third degree AV block, sinus node dysfunction, atrial condition defects, cardiogenic shock unless a ventricular programmable pacemaker is present to sustain rhythm
  • have chronic atrial fibrillation
  • have digitalis intoxication induced arrhythmia
  • have asymptomatic or mildly symptomatic ventricular arrhythmias
  • have severe bradycardia or pronounced hypotension
  • are taking medicines that interact with Flecarite, including other antiarrhythmics like amiodarone, disopyramide and verapamil, cimetidine for reflux, the antipsychotic phenytoin, the anticonvulsants carbamazepine and phenobarbitone, antidepressants like paroxetine and fluoxetine, antihistamines like terfenadine, antivirals like ritonavir for HIV, beta blockers like propanolol

What medications interact with Flecarite?

Several medications interact with Flecarite Tablets 100mg and should either not be taken while you are taking Flecarite or only after discussion and instruction from your doctor:

  • Drugs that cause a serious reaction with Flecarite: other antiarrhythmics like amiodarone, disopyramide and verapamil, antihistamines like terfenadine, beta blockers like propanolol
  • Drugs that affect Flecarite: cimetidine for reflux, , the antipsychotic phenytoin, the anticonvulsants carbamazepine and phenobarbitone, antidepressants like paroxetine and fluoxetine, antivirals like ritonavir for HIV

How should Flecarite be taken?

You should take your Flecarite Tablets 100mg swallowed whole with a glass of water, before or after meals, twice daily at the same time each day, morning and evening.  The dose you take and for how long depends on what you are being treated for and your doctor’s recommendation.  You should continue to take your Flecarite Tablets 100mg for as long as recommended by your doctor usually for at least four days to be effective.

Missed dose of Flecarite

If you miss a dose of Flecarite tablets 100mg take it as soon as you remember, unless it is time to take the next dose, then skip the missed dose.  Do not take a double dose.
