What is Protos used for?

Protos granules 2g are used to treat severe established osteoporosis in men and postmenopausal women, who are at high risk of fracture, to reduce the risk of vertebral and hip fractures, which can be painful and cause loss of height if the fracture occurs in the spine.  Osteoporosis is the loss of bone mass resulting in brittle bones which can fracture more easily and is often associated with lack of oestrogen in postmenopausal women.  Risk factors for postmenopausal osteoporosis include low bone mass and density, early menopause, hysterectomy, a history of smoking and a family history of osteoporosis.  Also low levels of testosterone can cause loss of bone density.  Protos granules 2g should only be used when other medications for the treatment for osteoporosis cannot be used or are considered unsuitable, due to contraindications or intolerance and taking into consideration various risk factors due to the medication and risk of fracture.  Protos granules 2g help to strengthen bone structure and increase bone density making bones less likely to fracture and reducing the risk of fracture.

How does Protos work?

Protos granules 2g contain strontium ranelate, a chemical similar to calcium that influences bone turnover and works by increasing bone formation and reducing bone resorption.  Bone turnover is a normal part of bone remodelling, which is a dynamic process that allows bones to grow and repair.  This process of bone turnover is hormonally controlled and is increased by oestrogen and decreased by corticosteroids.  Osteoporosis occurs when more bone is lost by bone resorption than new bone is formed, resulting in a breakdown of bone structure and loss of bone density and mass, making bone more susceptible to fracture.  Bone is comprised of bone forming cells, the osteoblasts and bone resorbing cells, the osteoclasts.  Studies have shown that strontium ranelate in Protos granules 2g stimulates osteoblast activity and inhibits osteoclast activity and that this effect on bone remodelling is linked to activation of calcium-sensing receptors on both cells types.  Other effects of strontium ranelate in Protos granules 2g on bone cells is stimulation of collagen production, as well as other proteins by osteoblast cells, and inhibition of enzymes that degrade cartilage; also reduction of calcium loss from bone.  All these actions of strontium ranelate in Protos granules 2g help support the structure of bone and improve bone strength. The overall effect of strontium ranelate in Protos granules 2g is to improve bone mineral density and promote a rebalance of bone turnover in favour of bone formation, which reduces risk of fracture.  

What does Protos contain?

Protos suspension granules contain the active ingredient strontium ranelate 2g, used to treat severe established osteoporosis in men and postmenopausal women.  They also contain aspartame, maltodextrin and mannitol.

What are the side effects of Protos?

The most commonly reported side effects when taking Protos granules 2g include headache, disturbances in consciousness, memory loss, insomnia, dizziness, paraesthesia (tingling pins and needles), vertigo, myocardial infarction, venous thromboembolism , bronchial hyperreactivity, gastrointestinal upset (nausea, diarrhoea, vomiting, abdominal pain, reflux, dyspepsia (heartburn) constipation), skin reactions (rash, pruritus, urticarial), eczema, hypercholesterolaemia (high blood cholesterol), musculoskeletal pain (muscle spasm, myalgia, bone pain, arthralgia and pain in extremity)

When should Protos not be used?

You should not use Protos granules 2g if you:

  • are allergic to strontium ranelate or any ingredients in Protos
  • are pregnant or are breastfeeding
  • have severe kidney disease
  • have established, current or past history of ischaemic heart disease, peripheral vascular disease, cerebrovascular disease, uncontrolled hypertension, venous thromboembolism, pulmonary embolism
  • are temporarily or permanently immobilised (e.g. post-surgical recovery or prolonged bed rest)
  • have any signs of a serious skin reaction, Stevens-Johnson syndrome, that can develop within the first few weeks of treatment and can be life-threatening
  • are taking medicines that interact with Protos, including foods (like milk, cheese ) and medicines containing calcium within 2 hours, tetracycline antibiotics like doxycycline and quinolone antibiotics like ciprofloxacin

What medications interact with Protos?

Several medications interact with Protos granules 2g and should either not be taken while you are taking Protos or only after discussion and instruction from your doctor:

  • Drugs that affect Protos: foods (like milk, cheese ) and medicines containing calcium within 2 hours, as they may reduce the bioavailability of strontium ranelate
  • Drugs that are affected by Protos: tetracycline antibiotics like doxycycline and quinolone antibiotics like ciprofloxacin

How should Protos be taken?

You should take your Protos granules 2g once daily swallowed after mixing in 30mL water (a third of a standard glass) to form a suspension. You should take your Protos granules in one dose on an empty stomach, two hours before or after a meal and preferably at bedtime and at the same time each day.  You should take your Protos granules 2g for as long as recommended by your doctor, which may be for a long period.

Missed dose of Protos

If you miss a dose of Protos granules 2g take it as soon as you remember, unless it is time to take the next dose, then skip the missed dose.  Do not take a double dose.
